Countdown to the 6th Annual
“Give Kids a Smile Day” on February 1, 2008!

Give Kids a Smile Day is a nationwide day of free dental treatment and education for underserved children. This event not only provides dental care for deserving kids, but also sends a message to policymakers that these children shouldn't have to depend on charity for basic dental attention.

Thank you to those dental offices that have registered to treat patients in your private practices, give school presentations or volunteer to treat patients in a nearby participating clinics.

IMPORTANT INFORMATION FOR GKAS VOLUNTEERS

Please feel free to download the following forms for your Give Kids a Smile Friday:

  • Give Kids a Smile Day Superbill.  If you are treating patients, please make copies of this superbill and fill one out for each patient you treat on Give Kids a Smile Day. Please mail or fax these forms back to the CDA for record keeping purposes (patients will not be charged for any services you provide). Forms can be faxed to 303-740-7989. This is the ONLY method the CDA has to record and track how much treatment was donated on Give Kids a Smile Day. Please send us this treatment tracking superbill so we can have an accurate report of Colorado’s Give Kids a Smile Day activities and donated services.
  • Patient Appointment Reminder Letter.  If you are providing treatment in your office for Give Kids a Smile Day, we strongly encourage you to remind your patients of their appointments.  Programs of this nature typically produce a higher than normal number of “no show” patients.  The enclosed letter has been produced to provide your office with a tool to help minimize “no shows.”  This letter can be filled out and mailed to patients.  You are also welcome to modify this letter for your office’s use.
  • Kids in Need of Dentistry (KIND) Information Flyer:  Please make copies of this English/Spanish, two-sided flyer and give it to each of your patients, regardless of whether or not they need additional care.  This is a resource to help them later, should they need dental care in the future.
  • Give Kids a Smile Day School Presentation Tracking Form: If you are visiting schools/community centers to provide education about good oral health, please fill this form out with the appropriate information.  The CDA would like to keep track of the schools and areas of the state that have received dental education for future educational outreach purposes.  Completed forms can be faxed to the CDA at 303-740-7989.

The ADA Selects Colorado to Host the National GKAS Press Event!

CU School of Dental Medicine Map and Parking

The CDA is proudly hosting the national GKAS press event for 2008. For this reason, we want to make GKAS bigger than ever before in Colorado. If you are still deciding on your participation activity, please consider volunteering to treat patients for GKAS at the CU School of Dentistry, where we will be hosting several of the ADA officers, corporate event sponsors and media. We provide the patients, we provide the instruments, we even provide dental students to assist you if you’d like.  If you are interested in volunteering at CU, please call Molly Pereira at the CDA to register, 303-996-2844 or 800-343-3010.
